Steps To Become A Patient
Through Insurance
#1 : Referral
To bill insurance we require a referral which includes:
Order for Occupational Therapy
Our Practice Name; Foundations Holistic Therapy
Diagnosis codes which we will be treating
#2 : Submit Intake Paperwork
Submit your email to receive an invitation to our online portal to complete the patient paperwork which is required to make an initial evaluation appointment. Once sending in your email please allow 24 working hours to receive the invitation.
#3 Submit Insurance Card
We will need a copy of your current insurance to have for our records. Please be in direct contact with your own insurance company regarding your benefits.
#4 Schedule Evaluation
Following the completion of the previous steps we will schedule your initial evaluation and when appropriate your following therapy sessions.
